General practices are being asked to focus on care for people with moderate risk. People triaged as moderate risk through the National Coronavirus Helpline and COVID-19 symptom checker are advised to seek care from their GP. The vast majority of care will be delivered by telehealth.

GPs should refer to their local HealthPathways for up-to-date information on the clinical management of these patients, including escalation pathways if people deteriorate and referral to disease modifying treatments such as monoclonal antibodies.

General practice support

Current events have emphasised the importance of having text-based message alert system for general practices in our region. We are establishing a free subscription-based text message alert system that will send emergency and public health messages from the PHN directly to GP’s phones.

Subscription to this service will also include access to the Emergency Response Planning Tool (ERPT).

For practices who have more one or more GPs subscribing to the alert system, the PHN will reimburse subscriptions to the ERPT (this includes practices who already have access to ERPT).

For reimbursement, email an invoice to practicesupport@ddwmphn.com.au with the heading ERPT reimbursement and include a copy of your subscription invoice and the details of the GPs subscribing to the alert system.

Telehealth

The Department of Health has extended the healthdirect Video Call COVID-19 GP Program to 30 June 2022.  We encourage you to continue using this secure video consulting platform which has been purpose-built for primary care settings.

General Practice After Hours Support Program

To support general practice and the current increased patient demand for services, the PHN is offering payments and practice performance support tools for general practices to extend their opening hours into the after hours period in order to:

  • Address the additional demands currently being experienced across the health system and our communities.
  • Support the ‘Living with COVID-19’ health care environment moving forward.
  • Help optimise current practice resources, with a view to providing ongoing after hours services.

National COVID-19 Clinical Evidence Taskforce

The Taskforce works to provide the best COVID-19 evidence that supports clinicians to provide the best possible care for Australians during the COVID-19 pandemic. The Taskforce undertakes continuous evidence surveillance to identify and rapidly synthesise emerging research in order to provide national, evidence-based guidelines for the clinical care of people with COVID-19.

Clinical service provider for home visits for COVID-19 positive patients

To support the management of COVID-19 positive patients in the community, services have been comissioned to complete home visits where a GP or nurse may not have capacity or when a person needs to be seen during the after hours period.

The home visits are not intended for routine monitoring (which can be delivered by telehealth in most instances), but to provide a face-to-face visiting service by exception. This will help avoid unnecessary escalation of patients to hospital.

Eligibility requirements

For a patient to be eligible for this service they must be COVID-19 positive and isolating at home as per Queensland Health’s Public Health Direction.

  1. The home visit is required to avoid an unnecessary escalation to hospital.
  2. The managing general practice is not able to deliver the home visit itself.

When a patient is no longer isolating at home, they are no longer eligible for this service.

This service does not include conducting PCR or RAT testing.
