Chronic Conditions services funded by the Darling Downs and West Moreton PHN have been realigned to the National Strategic Framework for Chronic Conditions. These new services replace the former Rural Primary Health Services (RPHS) Program. 

Benefits of new Chronic Conditions services include:

• a person-centred approach to care, giving clients more choice and control over the types of services that they access, while ensuring the service they access meets their health needs.
• equitable service delivery across our region, ensuring that individuals in each local government area have the opportunity to access the care they need.
• comprehensive care for people with multiple chronic conditions and enhanced support through an expanded health team working with your patients. 

Eligibility 

Eligibility for all of these services is people with a diagnosed chronic condition or risk factor, specifically:

• arthritis
• cardiovascular disease
• chronic pain
• diabetes
• obesity
• physical inactivity
• respiratory disease

Referral Pathway

GPs and allied health professionals can commence referring patients to the new Chronic Conditions services from 02 January 2019. Referrals can be made using the Darling Downs and West Moreton PHN Chronic Conditions Referral Form which can be faxed or sent through Medical Objects to providers.
