The Darling Downs and West Moreton PHN has been established as a commissioning organisation and ordinarily will not directly deliver health nor medical services. Commissioning is intended to assist in meeting our objectives of improved efficiency, effectiveness and coordination of primary health care.
We focus on resolving service provision gaps in a cost-efficient manner. In many cases, these gaps, once revealed, can be filled by negotiation with existing providers. In some cases, we made need to bridge a primary healthcare gap, funding new services.
The procurement of services is only one outcome to the commissioning process. Commissioning relies on robust relationships and established trust with stakeholders to:
design the appropriate response and
monitor and evaluate the outcomes.
The Darling Downs and West Moreton PHN strives to work closely with our stakeholders, including our Clinical Councils and Community Advisory Councils to establish local priorities and plan suitable solutions to ensure the best possible patient outcomes.
Darling Downs and West Moreton PHN, in collaboration with Diabetes Queensland, Stroke Foundation, Queensland Aboriginal and Islander Health Council, Ethnic Communities Council of Queensland and the Heart Foundation, are looking for Expressions of Interest (EOI) from General Practices and Allied Health organisations to become provider organisations for My Health for Life. Funded by the State Government and delivered by the Healthier Queensland Alliance, the program encourages and supports participants to achieve and maintain healthy lifestyle goals.
ABOUT MY HEALTH FOR LIFE
My Health for Life is a Queensland integrated risk assessment and behaviour modification program that aims to assist participants to reduce their risk of developing chronic conditions such as type 2 diabetes, heart disease and stroke. Based in the community and delivered by local healthcare practitioners, this initiative will identify those at high risk of developing chronic conditions and offer them a tailored, but flexible, healthy lifestyle program.
HOW DOES IT WORK
My Health for Life will offer eligible participants a structured six-month lifestyle modification program. This program works in partnership with GPs, providing patients an opportunity to participate in an evidence-based lifestyle modification program in their local community. Facilitators are trained health professionals, including dietitians and exercise physiologists, who have a keen interest in preventive health.
This program is a practical extension of the advice given by GPs to their patients, allowing participants to better understand their lifestyle risks and action their health goals. GPs will be kept informed of their patient’s progress, completing the care cycle. Health professionals are a critical part of the My health for life program, and we look forward to building strong, professional relationships while contributing to your patients’ overall care.

Interested provider organisations are requested to complete the Expression of Interest for the Darling Downs Region and return to firstname.lastname@example.org by COB Monday 2 October 2017. Access the Provider EOI Pack for Darling Downs for more information.
Please note this is not a tender process, multiple organisations will be approved to deliver the program from the same region.
Once all EOIs are received they will be assessed by the My health for life Operations Management Committee. All applicants will be provided with a response to their EOI via return email by Friday 13 October.
Successful organisations will then need to identify appropriately qualified staff to attend facilitator training on 9 & 10 November 2017 (location to be confirmed).
