One of the Darling Downs and West Moreton PHN’s key activities, is to understand the healthcare needs of our communities. We achieve this via research, consultation, validation, analysis and planning, which assists us to identify and address primary care service gaps.

The development of a Health Needs Assessment (HNA) annually, examines the health of our community, the services that are available and any gaps that exist, which assists us in determining the key clinical and service integration priorities.
