The Darling Downs and West Moreton PHN have entered into a collaborative partnership with the Darling Downs Hospital and Health Service to deliver Darling Downs HealthPathways.
The Darling Downs HealthPathways Project aims to establish a web-based information portal supporting primary care clinicians plan patient care through our primary, community and secondary health care systems with localised referral pathways for the Darling Downs region.
HealthPathways are designed to be used at the point of care, primarily for General Practitioners but is also available to Hospital Specialists, Nurses, Allied Health and other Health Professionals within our region.
Darling Downs HealthPathways is looking to establish a Professional Advisory Network to contribute to decisions relating to the planning, delivery and evaluation of the Darling Downs HealthPathways Program.
The Professional Advisory Board plays an important role in providing expert knowledge, understanding and strategic thinking within community and primary health care services.
Members of the network will provide insights and guidance relevant to improving health services delivery across the health care continuum and will work closely with the Clinical Director, General Practice Clinical Lead and the Darling Downs HealthPathways (DDHP) Team.
