The Darling Downs and West Moreton PHN have entered into a collaborative partnership with the Darling Downs Hospital and Health Service to deliver Darling Downs HealthPathways. 

The Darling Downs HealthPathways Project aims to establish a web-based information portal supporting primary care clinicians plan patient care through our primary, community and secondary health care systems with localised referral pathways for the Darling Downs region. 

HealthPathways are designed to be used at the point of care, primarily for General Practitioners but is also available to Hospital Specialists, Nurses, Allied Health and other Health Professionals within our region.
