Clinical service provider for home visits for COVID-19 positive patients

To support the management of COVID-19 positive patients in the community, services have been comissioned to complete home visits where a GP or nurse may not have capacity or when a person needs to be seen during the after hours period.

The home visits are not intended for routine monitoring (which can be delivered by telehealth in most instances), but to provide a face-to-face visiting service by exception. This will help avoid unnecessary escalation of patients to hospital.

For a patient to be eligible for this service they must be COVID-19 positive and isolating at home as per Queensland Health’s Public Health Direction.

  1. The home visit is required to avoid an unnecessary escalation to hospital.
  2. The managing general practice is not able to deliver the home visit itself.

When a patient is no longer isolating at home, they are no longer eligible for this service.

This service does not include conducting PCR or RAT testing.
