Chronic Conditions Contracts Awarded

Darling Downs and West Moreton PHN is pleased to announce that contracts have been awarded to service providers to deliver Chronic Conditions services across the Darling Downs and West Moreton region from 02 January 2019.    

Chronic Conditions services funded by the PHN have been realigned to the National Strategic Framework for Chronic Conditions replacing the former Rural Primary Health Services (RPHS) Program.

Funding for this priority has not been reduced, however services have been realigned to include preventative services and care coordination and maximise PHN resources to ensure equitable service delivery across the region

New services supporting people with chronic conditions will place the patient at the centre of care, giving individuals more choice and control over their treatment options with the support of their GP. This will include comprehensive care for people with multiple chronic conditions and enhanced support through an expanded health team working with your patients.

New services by Local Government Area (LGA) 

 PHN-funded providers will commence delivering services from 02 January 2019. Group activities will commence following 02 January 2019 once referrals have been received for these programs.

Eligibility 

 Eligibility for all Chronic Conditions services is people with a diagnosed chronic condition or risk factor, specifically:

  • arthritis
  • cardiovascular disease
  • chronic pain
  • diabetes
  • obesity
  • physical inactivity
  • respiratory disease

Referral Pathway

GPs and allied health professionals can commence referring patients to the new Chronic Conditions services from 02 January 2019. Referrals can be made using the PHN Chronic Conditions Referral Form which can be faxed or sent through Medical Objects to providers. This information will be made available via our Interactive Services Map on 02 January 2019.
