Darling Downs and West Moreton PHN are in the process of realigning Chronic Disease services funded by the PHN with the National Strategic Framework for Chronic Conditions.
To ensure continuity of support for clients in our communities, existing contracts with Chronic Disease service providers have been extended until the 31st of December 2018. A re-commissioning cycle has commenced to identify and embed providers across the region from the 1st of January 2019. These providers will deliver services for people with chronic conditions replacing the former Rural Primary Health Services (RPHS) program funded by the PHN. The services will focus on:
We are now seeking applications for the below interventions:
Care Coordination: Care coordination services for clients with complex chronic conditions and other possible co-morbidities including mental health.
Individual Interventions from Health Professionals:
Individual interventions from Health Professionals including, but not limited to: physiotherapy, podiatry, exercise physiology, diabetes education and dietetics, occupational therapy, respiratory, speech pathology and groups interventions.
Telehealth Interventions
Multi-disciplinary Interventions
The services should target high incidence chronic conditions and/or risk factors, including:
