My health for life update - August

My health for life (MH4L) is a free behaviour change program designed for people at high risk of developing a chronic disease and shows participants that making small lifestyle changes can have major health benefits.

The program works in partnership with general practice and is a practical extension of the advice given by GPs and nurses to their patients. For more information visit www.myhealthforlife.com.au

My health for life working with GPs to prevent chronic disease

Did you know that GPs that refer patients into the MH4L program are provided with patient progress reports?   The My health for life program works in partnership with general practices, and is a practical extension of the advice given by GPs and nurses to their patients. GPs are kept informed when the patient commences and completes the program, and also if they withdraw. In the near future, GPs will also be informed when the patient is halfway through the program and what SMART goal the patient is working towards.

My health for life (MH4L) is a free behaviour change program available to eligible patients who want to improve their health and reduce their risk of developing chronic disease.

My health for life at your workplace – a healthier employee is a more productive employee!

We know that healthy employees record fewer sick days, are more productive and engage better with colleagues and customers. The My health for life team can support your staff to understand their health risks, and provide a program aimed at reducing the risk of chronic disease.

The My health for life team can provide:

  • Tools and resources to promote this initiative
  • On-site health checks
  • The My health for life program in the workplace
  • The My health for life group-based program in local communities
  • A telephone health coaching service, available statewide.

Remember, participants are also able to complete the program through telephone coaching if there are no local sessions available in your area.

Patients can be referred using the practice software MH4L referral template or calling 13 Risk (13 74 75).

Darling Downs West Moreton PHN can work with your practice to identify patients who are eligible for referral into the program. If you would like more information on how to get involved please contact your Primary Engagement Officer.
