My health for life working with GPs to prevent chronic disease
Did you know that GPs that refer patients into the MH4L program are provided with patient progress reports?
The My health for life program works in partnership with general practices, and is a practical extension of the advice given by GPs and nurses to their patients. GPs are kept informed when the patient commences and completes the program, and also if they withdraw. In the near future, GPs will also be informed when the patient is halfway through the program and what SMART goal the patient is working towards.
My health for life (MH4L) is a free behaviour change program available to eligible patients who want to improve their health and reduce their risk of developing chronic disease.
