West Moreton HealthPathways Synopsis

On Tuesday 7th August, the Darling Downs and West Moreton PHN in conjunction with West Moreton Health held a synopsis to introduce the West Moreton HealthPathways program to health professionals within the region.

West Moreton HealthPathways is an important collaborative initiative for both West Moreton Health and Darling Downs and West Moreton PHN. It is key to improving connections to referral services in our local health system to ensure timely care for patients.

What is HealthPathways?

West Moreton HealthPathways portal acts as a ‘care map’ that allows all members of a health care team, whether they work in a hospital or the community, to be on the same page when it comes to looking after a patient.

HealthPathways are designed to be used at the point of care, primarily for general practitioners, however it is also available to hospital specialists, nurses, allied health and other health professionals.

The HealthPathways contain information on how to assess, manage and refer a patient in the local context of available services with appropriate local referral and local pre-referral work up requirements, ensuring faster access for patients.

HealthPathways allows for the improved quality, safety and experience of care for patients by maximising the value of health system resources.

As a collaboration between the West Moreton Health and Darling Down and West Moreton PHN, the West Moreton HealthPathways portal empowers clinicians with locally agreed information to make the right decisions, together with patients, at the point of care.
