AGES-RaSS: specialised support for GPs caring for RACF residents

Acute Geriatric Evaluation Services – Residential Aged Care Support Service (AGES-RaSS) consists of experienced aged care nurses and doctors, who work in partnership with GPs, RACFs, hospital and health services and community service providers to ensure the right care is received in the right place, at the right time.

The AGES-RaSS team would welcome the opportunity to discuss their service with GPs who provide care for patients currently in RACFs.

Their team is available Monday to Friday 8:00am–6:00pm and Saturday to Sunday 8:30am-5:00pm.
