Call for nominations - Care at the End of Life Collaborative Awards

The West Moreton Region Care at the End of Life Collaborative is celebrating its five year anniversary in 2023. To mark this occasion, we want to recognise the hard work and dedication of those working and caring in West Moreton to improve care at the end of life for our community. This is an unmissable opportunity for you to join with us and celebrate our local Care at the End of Life superstars.

There are 8 award categories:

• Outstanding contribution to care at the end of life in the aged care setting
• Outstanding contribution to care at the end of life in the community / primary care setting
• Outstanding contribution to care at the end of life in the inpatient setting
• Outstanding contribution by a volunteer in care at the end of life
• Outstanding contribution to care at the end of life by an organisation
• Health student award for excellence in care at the end of life

• Community superstar award for contribution to those approaching end of life
• Outstanding leadership in progressing care at the end of life in West Moreton

Awards will be announced and presented at the Care at the End of Life Collaborative Forum being held at the Ipswich campus of the University of Southern Queensland on 9 February 2023. Award finalists will be invited to attend the Forum, and the presentations will occur at the end.

Nominations are open now and close on 24 December 2022
