West Moreton Health Update: Care At The End Of Life Resource Launch

West Moreton Health is partnering with healthcare providers across the region to improve the quality of care provided at the end of life.
As a local leader in palliative care and host of the West Moreton Health Region Care at the End of Life Committee, the health service will join its partners at the upcoming Flying in Formation Forum to launch the West Moreton Region Care at the End of Life Knowledge Framework.
The Knowledge Framework will be a user-friendly guide for health professionals and caregivers that presents what end of life health and social carers need to know as core business in their roles while offering easy access to education opportunities and resources, helping to build the knowledge and capacity of those caring for people at the end of life in the West Moreton community.
The Framework is a guide rather than a formal education program and is not a mandatory requirement for health or social care professionals to be able to care for people at the end of life.
It will be implemented across the region during September and October 2019, with interested health and social care organisations invited to become early implementation sites.
Copies of the Knowledge Framework will be available at the Flying in Formation Forum which will also include presentations and panel discussions from experts in the field of palliative care from around the West Moreton region.
