Developing a regional West Moreton Older Persons Services Strategy and Five-Year Action Plan

Did you know that there will be at least 65% more people aged 65 or older living in the West Moreton Region by 2026 when compared to 2018?
This represents the largest percentage growth of any age group within both our region and also Queensland! With this in mind, it is important we recognise the care of older persons as part of our core business across our Hospital and Health Service and also the broader care continuum. West Moreton Health has developed an Older Persons Care Framework which is intended to inform our planning and change initiatives, and shape relevant services to meet the specific needs of older persons both now and into the future – click here to access the framework.
Following the development of the framework, the Darling Downs and West Moreton PHN (PHN) have committed to creating a West Moreton Older Persons Services Strategy and Five-Year Action Plan (The Strategy) in partnership with West Moreton Health in 2021. It is intended that The Strategy will define the regional population health priorities and outline strategies and actions to address these, including the recommendations of the Royal Commission into Aged Care. Additionally it is intended that The Strategy will guide the way we plan and deliver local health services and support older people in our region to stay healthy, connected and at home.
We seek input into the development of The Strategy and are investigating existing channels to enable collaboration.
