The PHN and West Moreton Health (WMH) are seeking Expressions of Interest from individuals wanting to be a part of the West Moreton Older Person’s Care Collaborative to share their experiences with WMH’s health services. The Collaborative will meet on Tuesday, 23 February 2021.
Darling Downs and West Moreton PHN and West Moreton Health are committed to establishing a West Moreton Older Persons Care Collaborative in 2021, with all relevant stakeholders and partners within the region.
Be part of creating a sustainable, integrated, outcomes focused, value-based healthcare service as the population of older people in West Moreton increases. Join us and share your experiences of West Moreton’s health services on February 23, 2021.
