Join the care finder community of practice

We’re looking for a consumer representative from the Darling Downs and West Moreton regions to join the care finder community of practice.The care finder community of practice will provide a space for funded care finder service providers and other representatives to share their experiences and learnings of the new care finder program.As a consumer representative for the community of practice, you will support continuous improvement through collaboration and responding to the evaluation of the program and adapting to the needs and trends within the target population.Requirements:

• Attend bi-monthly meetings via Teams
• Have an interest in services for older people
• Share care finder information with your community and/or networks.
