Major milestone for both HealthPathways teams in the region

The Darling Downs and West Moreton HealthPathways portals have both reached a significant milestones – each hitting over 800+ live pathways this January.

HealthPathways is a long-term program aimed at supporting GPs and health care professionals to guide their patients to better care. The service provides evidence-based information on the assessment and management of a range of clinical conditions – including clear referral guidance for local hospitals, specialists and allied health.

The web-based portal is designed to support clinicians guide their patients through the local public and private health system.

These pathways are developed collaboratively by local GPs, hospital clinicians and a wide range of other healthcare professionals.

The pathways represent the ongoing collaboration between the PHN and healthcare providers in the region, ensuring improved health outcomes through reduced wait times and reliable treatment guidance presented to clinicians alongside referral options in one place.

Among the most popular pathways were localised pathways, COVID-19 case management, medical, COVID-19, and surgical, viewed by 9,007 users in the West Moreton region, and 33,037 in the Darling Downs region.
