Making a Difference: Refugee Nurse Outreach Program

Darling Downs and West Moreton PHN works with culturally informed health services to make a difference by supporting our growing multicultural communities receive the appropriate health care they need.

The Refugee Health Nurse Outreach Program provides coordination for newly arrived refugees to access a GP and allied health services. Through the Program, refugee health nurses support general practice to conduct Refugee Health Assessments to improve health literacy, reduce barriers to ongoing health care management through linking refugees with other health services and facilitate the appropriate transfer of medical information to ensure timely and appropriate care.
