SPACE - Specialist Palliative Care in Aged Care

The purpose of the SPACE Project will enhance equitable access to specialist palliative care in Darling Downs Residential Aged Care Facility’s (RACF)
The benefits of the SPACE Project:
- Increase capacity and capability of General Practice and RACF staff
- Support the promotion of pre-emptive planning to avoid crisis management
- Provide specialist consultation to support management of residents with complex end of life needs
- Where clinically appropriate and where aligns to the residents wishes avoiding unnecessary transfers to hospital
The SPACE project aim is to provide education to RACF staff and implement Palliative Care Needs Rounds.
Palliative care needs rounds will identify those residents with a high symptom burden and or no end of life care planning in place. The Palliative Care Needs Rounds will be held monthly at each RACF or via telehealth and will run for approximately 90 minutes where four to six residents will be discussed.
