Activity 9 – Mental Health Nurse Care

Priority Area – Mental health services for people with severe and complex mental illness including care packages

This activity commissions primary mental health services for people with severe mental illness being managed in primary care, including clinical coordination for people with severe and complex mental illness through the phased implementation of primary mental health stepped care.

This will be achieved through a multi-faceted approach:

  • Commission mental health nurses and other services to support clinical care coordination for people with severe mental illness, ensuring service continuity to existing MHNIP clients and developing new services in areas with identified need.
  • Promote the use of a single care plan for people with severe and complex mental illness, to help link multiple providers and services involved in a client’s care.
  • Engage with the private mental health care sector to ensure links are in place with local hospitals and psychological services to support care coordination.
  • Ensure referral pathways are in place to enable and support patients to seamlessly transition between services as their needs change.
  • Complement existing GP, psychiatrist and allied mental health professional services available through the Medicare Benefits Schedule (MBS) and other providers including other funded government departments and nongovernment service providers.
