Patient Experience

The Darling Downs and West Moreton PHN is committed to working across our region to reinforce core strategies which drive improved patient experience. We understand that primary health care is critical and has an impact on the human experience in healthcare. 

Our practice support team supports our region by promoting the fundamentals in how we build, lead and operate our healthcare organisations and systems, now and into the future.

We are currently reviewing strategies to record and monitor patient reported experiences as a measure of health system performance and patient experiences and perceptions, in order to use these result as a tool to drive service integration and excellence in seamless patient care. 

In order to continually promote positive changes to patient experience the Darling Downs and West Moreton PHN has developed an implementation plan which includes projects aimed at supporting primary care professionals to improve the patient experience through shared electronic health records and measuring clinical outcomes.
