The Queensland Ambulance Service, Queensland Health, Darling Down and West Moreton Primary Health Network and GP networks are working together to try and meet the health needs of the Darling Downs and West Moreton community through implementation of Connected Community Pathways project.
The project aims to:
- reduce avoidable ED presentations to facilitate improved access and patient flow through the ED.
- reduce potentially preventable hospitalisations to facilitate more sustainable utilisation of inpatient capacity (beds).
- improve timely and appropriate access to care that is as close to home as possible where clinically appropriate.
Our goal is to get some feedback from you about your satisfaction with the pathway of health care which you accessed on this occasion. Completion of the survey is voluntary. All information collected in this survey is anonymous. None of the information collected will be used to identify you.
