Mental Health Nurse Care is for people who are diagnosed with a severe and complex mental illness in the Darling Downs and West Moreton region who are currently being managed in the primary care setting via a General Practitioner and/or Psychiatrist. People who are deemed eligible will receive mental health nurse services through a clinic setting or outreach model including clinical coordination of services by a mental health nurse. Targeted Psychological Therapies (TPT) replaces the former Access to Allied Health Psychological Services (ATAPS) and Mental Health Services in Rural and Remote Areas (MHSRRA). TPT is designed to complement MBS Better Access psychology services funded by Medicare, not be in place of these services. • Support (up to three sessions) can be accessed as a Provisional referral prior to having a Mental Health Treatment Plan if the referral is urgent (within 24-48hrs), low risk of suicide, or a priority i.e. for young children; • Referrals can also be made for patients to access MBS Better Access through this referral process (these services are funded through MBS item numbers); • After a patient has exhausted all of their MBS Better Access sessions (maximum 6+4 Mental Health Treatment Plan MBS), and it is clinically appropriate for additional time, limited support of Up to five supplementary sessions can also be provided; • Some TPT providers may deliver Groups where clinically appropriate and suited to the client. |

Over six private sessions, individuals have access to a coach who will help them to set practical and effective goals, giving them the skills to manage the pressures of life so that they can get back to feeling their best. Palliative Care After Hours - support, advice and outreach to the residents of Kingaroy and surrounds. This service will provide support to patients who require palliative care who wish to remain in their own home. Palliative care nurses will provide support, symptom management, After Hours care planning, outreach to home services and advice in the After Hours period while working closely with the patient's GP to provide support to patients and their carers. The palliative care service will be provided at no cost to the patient. |

Mental Health Nurse Care is for people who are diagnosed with a severe and complex mental illness in the Darling Downs and West Moreton region who are currently being managed in the primary care setting via a General Practitioner and/or Psychiatrist. People who are deemed eligible will receive mental health nurse services through a clinic setting or outreach model including clinical coordination of services by a mental health nurse. Mental Health Nurse Care will be delivered through: • face-to-face sessions |

Targeted Psychological Therapies (TPT) replaces the former Access to Allied Health Psychological Services (ATAPS) and Mental Health Services in Rural and Remote Areas (MHSRRA). TPT is designed to complement MBS Better Access psychology services funded by Medicare, not be in place of these services. • Support (up to three sessions) can be accessed as a Provisional referral prior to having a Mental Health Treatment Plan if the referral is urgent (within 24-48hrs), low risk of suicide, or a priority i.e. for young children; • Referrals can also be made for patients to access MBS Better Access through this referral process (these services are funded through MBS item numbers); • After a patient has exhausted all of their MBS Better Access sessions (maximum 6+4 Mental Health Treatment Plan MBS), and it is clinically appropriate for additional time, limited support of Up to five supplementary sessions can also be provided; • Some TPT providers may deliver Groups where clinically appropriate and suited to the client. |

Targeted Psychological Therapies (TPT) replaces the former Access to Allied Health Psychological Services (ATAPS) and Mental Health Services in Rural and Remote Areas (MHSRRA). TPT is designed to complement MBS Better Access psychology services funded by Medicare, not be in place of these services. • Support (up to three sessions) can be accessed as a Provisional referral prior to having a Mental Health Treatment Plan if the referral is urgent (within 24-48hrs), low risk of suicide, or a priority i.e. for young children; • Referrals can also be made for patients to access MBS Better Access through this referral process (these services are funded through MBS item numbers); • After a patient has exhausted all of their MBS Better Access sessions (maximum 6+4 Mental Health Treatment Plan MBS), and it is clinically appropriate for additional time, limited support of Up to five supplementary sessions can also be provided; • Some TPT providers may deliver Groups where clinically appropriate and suited to the client. |

The Strong Mind, Strong Spirit program is a culturally appropriate support program designed to reach out to those with or at risk of mental illness, their family and community. The program is facilitated by Aboriginal Mental Health Workers and is delivered in a group setting with one-on-one counselling offered if required. There are at least two group sessions per week in both Toowoomba and Warwick including a women’s and men’s group. |

The New Sensations program offers a one-day workshop followed by up to four one-hour individual therapy sessions. The workshop will introduces participants to Sensory Modulation and the individual sessions tailor strategies to meet an individuals needs. Sensory Modulation can be described simply as improving the way someone feels or functions by using their senses. Sensory Modulation can help people to self-regulate when facing challenging moods, emotions, thoughts and feelings, especially when talking or thinking may be difficult. Participants learn strategies to improve their mood, manage their stress, feel more calm and relaxed or more alert and focused, and more motivated to get things done. Sensory items will be provided to explore in the group and some small items will be provided to take home. |

Mental Health Nurse Care is for people who are diagnosed with a severe and complex mental illness in the Darling Downs and West Moreton region who are currently being managed in the primary care setting via a General Practitioner and/or Psychiatrist. People who are deemed eligible will receive mental health nurse services through a clinic setting or outreach model including clinical coordination of services by a mental health nurse. Services Offered |
Targeted Psychological Therapies (TPT) replaces the former Access to Allied Health Psychological Services (ATAPS) and Mental Health Services in Rural and Remote Areas (MHSRRA). TPT is designed to complement MBS Better Access psychology services funded by Medicare, not be in place of these services. • Support (up to three sessions) can be accessed as a Provisional referral prior to having a Mental Health Treatment Plan if the referral is urgent (within 24-48hrs), low risk of suicide, or a priority i.e. for young children; • Referrals can also be made for patients to access MBS Better Access through this referral process (these services are funded through MBS item numbers); • After a patient has exhausted all of their MBS Better Access sessions (maximum 6+4 Mental Health Treatment Plan MBS), and it is clinically appropriate for additional time, limited support of Up to five supplementary sessions can also be provided; • Some TPT providers may deliver Groups where clinically appropriate and suited to the client. |

The NewAccess Program is a free service developed by beyondblue that provides support in the form of a specially trained coach. Over six private sessions, individuals have access to a coach who will help them to set practical and effective goals, giving them the skills to manage the pressures of life so that they can get back to feeling their best. The NewAccess program is offered over the phone or face to face. |

The New Sensations program offers a one-day workshop followed by up to four one-hour individual therapy sessions. The workshop will introduces participants to Sensory Modulation and the individual sessions tailor strategies to meet an individuals needs. Sensory Modulation can be described simply as improving the way someone feels or functions by using their senses. Sensory Modulation can help people to self-regulate when facing challenging moods, emotions, thoughts and feelings, especially when talking or thinking may be difficult. Participants learn strategies to improve their mood, manage their stress, feel more calm and relaxed or more alert and focused, and more motivated to get things done. Sensory items will be provided to explore in the group and some small items will be provided to take home. |

Eligibility Criteria |
Hospice care is provided to anyone with a life-limiting health condition, who their doctor believes to be in the end-stage of their illness, needing a level of care that cannot be safely provided at home, or if the client does not wish to die at home. Clients may be referred to the Hospice by themselves, their families and friends, their GP, Community Service Providers and other hospitals.
