Integrated service delivery is essential so that clients receive a continuum of preventive and curative services, according to their needs over time and across different levels of the health system.  

The vision is to achieve a health care system where clients receive the right care, at the right time, in the right place.

The Darling Downs and West Moreton PHN is pro-actively working with primary health providers to empower integration of primary, secondary and tertiary health care that is seamless, smooth and easy to navigate. 

Across all programs and initiatives, we are seeking:

• avenues to develop and educate the health workforce around awareness of health pathways for patients as a whole (not just one clinical aspect), and 
• for all levels of the health workforce, to communicate well.
