Empower your patients to live well and thrive with APNA's Chronic Disease Management and Healthy Ageing Program
APNA's Chronic Disease Management and Healthy Ageing (CDMHA) program provides an online, evidence-based education framework to support nurses in primary health care to increase their knowledge, skills, and confidence in key chronic disease management and healthy ageing topics.
This six-month, intermediate-level program includes monthly workshops on key health priority areas. Workshop participants have access to self-assessment tools and online learning modules, and they are encouraged to share their learning, experiences, and progress through a community of practice.
The Australian Government subsidises the course, which costs just $220. Our next intake starts on Thursday, 24 October 2024. Visit our website to secure your spot.
CDMHA is part of the Nursing in Primary Health Care Program, an Australian Government initiative.
