Virtual care in acute and chronic disease management in the community.

West Moreton Health (WMH) invites you to join the MeCare team and Hospital in the Home team (HITH) for a Grand Rounds discussion about use of virtual care in managing acute and chronic diseases in the community.

The purpose of the Grand Rounds discussion is to:
1. To promote virtual care in acute and chronic disease management in the community.
2. To build relationships between primary and secondary health care within West Moreton to improve access to medical care at the comfort of patients’ home.
3. To explore the current model of care in the community in managing chronic medical illness.

More about the speakers:
Dr Danusha De Alwis has been a geriatrician at WMH since May 2020. She is the virtual care consultant for WMH. She works in the community three days a week as the Mecare consultant and the rest of the days she works as the consultant for Residential Aged care Support Service (RaSS) and HITH at Ipswich hospital.

Dr Lim is a geriatrician based at the Ipswich Hospital and has been the consultant for Hospital in the Home for the last 5 years. She has expanded this service to continue supporting the Ipswich community to receive inpatient care at home through the Hospital In the Home service. This service also supports the Home IV program for long term IV antibiotics and the infusion service.
