Darling Downs and West Moreton PHN is seeking a motivated individual who has a passion for primary health care and digital health to join our team as a Systems Integration Coordinator, based in Springfield or Toowoomba to cover a maternity leave vacancy.
Darling Downs and West Moreton PHN (PHN) is a not for profit organisation that works at a regional level to achieve improved whole of system health care.
Our key objectives are to:
- increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
- improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
Our PHN achieves this by working with local health care stakeholders and community to:
- fund programs and services based on local health needs;
- support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients; and
- support the development of a local sustainable health care workforce.
The Role Purpose:
This role is a 12 month contract to cover a maternity leave vacancy.
As the Systems Integration Coordinator you will be tasked with a wide variety of responsibilities that assist with the continual maturity of the organisation with key focus areas:
- Develop and execute system integration strategies across the health sector.
- Work across the organisation to streamline projects and priorities.
- Build successful relationships and partnerships with key stakeholders both internally and externally.
- Participate in regional planning, integration and implementation of the PHN’s Healthy Primary Care Plan.
The System Integration Coordinator role will work closely with health system stakeholders to support the coordination and delivery of the PHN’s GP Workforce Collective Impact Action Plan and Allied Health Engagement Framework. The System Integration Coordinator will work closely with the Manager of Integration to implement activities to support the development the regional health workforce including GPs, practice managers, practice nurse and allied health. The role will also develop and execute system integration strategies across the local health system to reduce potential avoidable hospitalisations and emergency department presentations.
The Role Responsibilities
Include establishing and maintaining productive working relationships with General Practices, GPs, practice managers, Allied Health, ACCHOs and local Hospital Health Services to:
- Plan and coordinate consultations and surveys with General Practice teams, Registrars, Allied Health and GPs in the PHN Region to seek feedback on local workforce recommendations.
- Plan and implement events and activities that support the attraction, retention and upskilling of the GP and allied health workforce including professional development opportunities for GPs, Practice Managers, Practice Nurses and Allied health professionals.
- Track, support and deliver initiatives outlined in the PHN’s GP Workforce Collective Impact Action Plan
- Support the delivery of initiatives outlined in the PHN’s Allied Health Engagement Framework
- Contribute to the implementation of the Medicare Reform Agenda Work collaboratively with GP Advisors, liaise with existing Registrars to determine suitability of the training placements, opportunities for gaining advanced skills, and other issues that may affect trainee wellbeing.
Selection Criteria
- Relevant experience in Health, Allied Health, Nursing Human services, public health or health service planning.
- Experience in effective stakeholder engagement, partnership brokerage and relationship development with the health community including consumers, carers, advocates, providers, policy makers and others.
- Understanding and working knowledge of workforce development in team care environments.
- Experience in a healthcare or human services environment, particularly with sector development models, resources, clinical governance, processes and tools to support and enhance system reform.
- Experience in program development and project management to affect systems change including a good understanding of data analysis, project evaluation and reporting.
- Highly developed interpersonal, communication and negotiation skills and the capacity to build and maintain relationships with DDWMPHN stakeholders
