The Integrated Team Care (ITC) Program aims to increase access to primary health care services for Aboriginal and Torres Strait Islander peoples. The program offers care coordination support for individuals with a chronic condition, and general support to access health services through outreach workers.

This support can assist patients to access:

  • affordable medicines
  • transport services to and from medical appointments
  • care coordination and supplementary services – coordinated multidisciplinary care for those living with chronic disease
  • other available community services.

These services are delivered by Aboriginal Medical Services (AMSs) and Aboriginal Community Controlled Health Organisations (ACCHOs) across our region.

Our funded ITC providers are:

  • Carbal Medical Services
  • CRAICCHS
  • Goondir Health Services
  • HealthWise
  • IUIH - through a partnership with Kambu Health.

Who is eligible?

For care coordination and supplementary services, patients must:

  • identify as Aboriginal and/or Torres Strait Islander
  • have a current 715 Health Check and GP Management Plan
  • have a referral from GP
  • have a diagnosed chronic condition.

For services with an Indigenous Outreach Worker, patients must:

  • identify as Aboriginal and/or Torres Strait Islander
  • require assistance to attend a Medical Appointment.

Who can refer?

  • For care coordination and supplementary services, patients need a referral from a GP.
  • For services with an Indigenous Outreach Worker, referrals can be made by a GP, Aboriginal Health Worker or through self-referral.
