Thriving Lockyer Kids
Thriving Lockyer Kids is a place-based initiative that supports children and families to thrive. First 2000 Days
We recognise that the first 2000 days of a child’s life, spanning from preconception through to starting school, are a critical period for development. The PHN works in alignment with our cross-system partners to deliver and coordinate programs that make a difference in the first years of life.
GP Shared Care
The PHN works in partnership with local primary care to help strengthen antenatal care pathways. Each year, the PHN hosts the Maternity Symposium which features expert speakers. Presentations from the last Symposium can be accessed via the PHN’s GP Portal.
Children and young people in out-of-home care

Early Years and Young Persons’ Health Strategy 2025-2028
The Early Years and Young Persons’ Health Strategy 2025–2028 outlines our commitment to giving every child and young person in the Darling Downs and West Moreton region the strongest possible start to life. With more than 9,000 babies born in our region each year and a population that is growing and rapidly changing, this strategy sets a clear direction for how we will work with families, primary care, community organisations and system partners to improve health outcomes from preconception through to early adulthood. Grounded in community voice and local data, it highlights the need for well-coordinated, culturally safe and accessible care that supports families at every stage of the journey.
Developed in partnership with health, education and community stakeholders, the strategy reflects what families and young people told us they need most: continuity of care, better navigation of services, stronger local support, and early intervention that is responsive to the challenges they face. By focusing on healthy communities, strong primary care, effective partnerships and a child-safe organisational culture, this strategy sets out a roadmap for long-term, sustainable change. Together with our partners, we are investing in a connected, equitable and thriving health system that empowers our youngest community members to lead healthy lives now and into the future.
