Our two HealthPathways websites contain up-to-date, localised guidelines and clinical pathways to support you in managing your patients. The HealthPathways website for your area is your first port of call for clinical and referral information.
Referring your patients to PHN funded services
For any PHN-funded service requiring a GP referral, please use our electronic referral system, refeRHEALTH.
Using refeRHEALTH ensures individual’s consent has been provided to allow our PHN to capture the minimum de-identified dataset required by the Department of Health and Aged Care to ensure ongoing funding of these services.
refeRHEALTH also allows referrers to see the up-to-date list of available providers and services.

- If you have not been registered, visit https://ddwm.referhealth.com.au/ to register
- On your dashboard, select ‘Make Referral’
- Select the service required and complete and submit referral
- Send a copy of the Mental Health Treatment Plan (if applicable) to the provider you have selected via secure messaging.

- Choose a location on your hard drive (e.g., Desktop or My Documents) and click ‘Save’.
Note: Do not open the template in Word if you plan to import it into clinical software.

Referring your patients to public hospitals in Queensland Health
Hospitals and Health Services across the Darling Downs and West Moreton region use GP Smart Referrals as their preferred method for receiving referrals from general practices
What is GP Smart Referrals?
GP Smart Referrals are electronic referral templates for Queensland Health Outpatient Specialties that streamline the referral process by auto-populating patient demographics and clinical records. Integrated with Best Practice and Medical Director software, they align with state-wide referral guidelines to ensure essential information is included, reducing the need for follow-ups and speeding up triage.
