The charter of the PHN network is to better understand the health needs of the Darling Downs and West Moreton PHN community through comprehensive, ongoing analysis and planning, in collaboration with the general practice, the Primary Health sector, Hospital and Health Services, the social sector, and the community. In understanding health needs across the community, the Darling Downs and West Moreton PHN is able to effectively commission services to meet those needs.
The PHN commissioning environment provides greater opportunity to design and co-design new forms of service delivery and enhanced opportunities to explore more collaborative ways of working with and engaging with communities, clinicians, service providers and other stakeholders. The Darling Downs and West Moreton PHN commissions services based on the needs, opportunities, priorities and options identified in the needs assessment.
The Darling Downs and West Moreton PHN's commissioning framework is based on best practice and international principles. It uses a co-design model, ensuring funding is delivered in a transparent, evidence-based and rigorous manner.
