Join West Moreton Health on Saturday 19 July (9am to 2pm) at the Brookwater Golf and Country Club for a free event focused on improving outcomes for patients with heart failure.
Led by West Moreton Health experts, the program includes sessions on 2025 heart failure management, guideline-directed medical therapy (GDMT), renal impairment, palliative care and a panel on community heart failure services.
It’s a great opportunity to connect with local specialists, share case-based insights, explore practical approaches to collaborative care and strengthen referral networks.
