How best to support patients with their Advance Care Plans

Join us for this co-delivered webinar by ACRRM and the Australian Digital Health Agency (ADHA). The presenters will discuss the importance of advance care planning, and the resources available to assist you in starting this conversation with your patients.

Given that practice software is also undergoing enhancements to enable health care providers to upload Advance Care Directives to a patient’s My Health Record, we will also be joined by an ADHA staff member who will demonstrate how advance care information may be uploaded by a clinician to the patient’s My Health Record during the session.　 There will be time available at the end of the session for questions and answers.
