SearchThe Elder Care Journal was developed by local Ipswich Elders with support of Kambu Health and Darling Downs and West Moreton PHN.
The journal supports Elders to keep track of their health information; including appointments, blood sugar and pressure record keeping. It also contains helpful information about health services, aged care support and digital health.
Elders co-designed all elements of journal, from layout and design to print production. Local Aboriginal artist Anthony Kemp created the artwork which represents the journey of an Elder moving from place to place within their community.
Ordering
The journal is available for free to Aboriginal and Torres Strait Islander peoples, Elders, and other community members who would benefit from this helpful resource.
