Darling Downs and West Moreton PHN is a not for profit organisation that works at a regional level to achieve improved whole of system health care.

Our key objectives are to:

  • increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
  • improve coordination of care to ensure patients receive the right care, in the right place, at the right time.

Our PHN achieves this by working with local health care stakeholders and community to:

  • fund programs and services based on local health needs;

  • Data analysis and interpretation: Work with internal stakeholders to understand data needs; identify appropriate methodologies, statistical techniques and data solutions required for analysis; identify trends, patterns and anomalies; develop insights that have meaning aligned with business decisions and strategy.
  • Reporting and visualisation: Create reports, dashboards, and data visualisations to ensure insights are clearly and concisely articulated to the audience. Communicate complex data insights to both technical and non-technical audiences. Identify opportunities to enhance data quality, reporting processes and self-service analytics. Ultimately, ensure that data products are fit-for-purpose.
