Expression of Interest: Childhood Immunisation Reminder Program

The PHN invites general practices across the Darling Downs and West Moreton region to submit an Expression of Interest (EOI) to participate in the Healthily Childhood Immunisation Reminder Program.
Delivered in partnership with Healthily, the fully funded program supports improved childhood vaccination rates through proactive reminders, outreach and multilingual resources, at no cost to your practice and with minimal staff time required.
About the program:
- Healthily works directly with your Practice Manager or Nurse during a single phone call to identify eligible patients who are overdue for childhood vaccinations.
- Parents and guardians of eligible patients will receive an SMS with easy-to-understand, multilingual information about childhood vaccinations.
- Families who do not respond to the SMS will receive a friendly AI voice call in their preferred language, helping ensure no one is missed due to language barriers or a busy schedule.
The program seeks to:
- Increase awareness of childhood immunisations among families.
- Improve childhood vaccination rates.
- Support culturally appropriate communication through multilingual resources.
- Contribute to better health outcomes across our diverse communities.
