Healthy Ageing Support helps connect older people, as early as possible, to the primary care, community and at-home services required so they are able to remain living in their own home and community for as long as possible. These services aim to support the older erson to retain their independence in their usual activities of daily living. Eligibility is for people 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people.
