Mental health nurses work with GPs and/or psychiatrists to provide coordinated clinical care for people with severe and complex mental illness. MHCCC is delivered in line with a GP Mental Health Treatment Plan or equivalent, developed by the GP or psychiatrist and provides support to review and monitor mental and physical health, monitor medication, liaise with health professionals and link individuals to other needed services.
Important note: this service is available to young people aged 16+, and adults.
