Chinchilla Medical Practice, in partnership with AH Diabetes, provide a multidisciplinary patient centred approach to management of diabetes and other chronic conditions to provide preventative interventions to a high-risk population. The target population are all people living with diabetes, those at risk of developing diabetes, and other chronic conditions.
