The Integrated Team Care (ITC) Program aims to increase access to primary health care services for Aboriginal and Torres Strait Islander people. The program offers care coordination support for people with a chronic condition, and general support to access health services through outreach workers. For more information, click here.

Eligibility Criteria

Care Coordinator and Supplementary Services:

• Identify as Aboriginal and/or Torres Strait Islander

• Have a current 715 Health check and GP Management Plan

• Have a referral from GP

• Have a diagnosed chronic condition

Indigenous Outreach Worker:

• Identify as Aboriginal and/or Torres Strait Islander

• Require assistance to attend a Medical Appointment
