To provide Speech Pathology services to children 0-17 years and adults 18+ .

Eligibility Criteria

To be eligible clients:

  • must have a GP Management Plan and an individual treatment plan (including focused on outcomes);
  • must be unable to access these services through MBS funding or privately;
  • are not eligible for other Darling Downs and West Moreton PHN funded services;
  • require lifestyle modification and self management support;
  • require more allied health assistance; and 
  • reside in the catchment are of the Service Provider.

Eligibility Criteria

Hospice care is provided to anyone with a life-limiting health condition, who their doctor believes to be in the end-stage of their illness, needing a level of care that cannot be safely provided at home, or if the client does not wish to die at home.

Clients may be referred to the Hospice by themselves, their families and friends, their GP, Community Service Providers and other hospitals. For more information about referring a client please contact our Director of Nursing.

Hospice Based Palliative Care located at 37 Chermside Road, Eastern Heights, Ipswich.

Eligibility Criteria
