Eligibility Criteria

have a diagnosed chronic condition or risk factor, specifically:

  • chronic pain (any area) for more than 3 months (minimum) of 3/10
Patients must also be aged between 15 - 75. 
 
Patients are excluded if they have any of the following:  
  • severe psychiatric illness
  • current workcover or accident insurance claim
  • current legal claim regarding health condition
  • cancer related pain/ palliative care
  • red flags

Eligibility Criteria

Patients must have exhausted all other funding options prior to using Darling Downs and West Moreton PHN Chronic Conditions Program funding. This includes their private health insurance, Department of Veterans’ Affairs, Medicare and NDIS funding. 

AND

  • have a diagnosis of chronic obstructive pulmonary disease or other respiratory conditions such as bronchiectasis, interstitial lung disease, chronic asthma or pulmonary hypertension
  • be recovering from an acute exacerbation of their condition
  • be willing to participate (even if they are on long-term oxygen therapy or are current smokers).

 Patients are excluded if they have any of the following: 

  • severe cognitive impairment
  • severe psychotic disturbance
  • a relevant infectious disease
  • musculoskeletal or neurological disorders that prevent exercise
  • unstable cardiovascular disease (e.g. unstable angina, aortic valve disease, unstable pulmonary hypertension)
  • unstable angina* or myocardial infarction during the previous month
  • resting heart rate > 120 beats / min after 10 minutes rest (relative contraindication)
  • systolic blood pressure > 200 mmHg ± diastolic blood pressure > 100 mmHg (relative contraindication)
  • resting pulse oximetry (SpO2)% < 85% on room6 air or while breathing the prescribed level of supplemental oxygen. The referring doctor should be notified and exercise assessment should not proceed.6 Suggest the patient should be referred for assessment for long-term oxygen therapy (LTOT) if not already receiving supplemental oxygen. See testing protocol for oxygen supplementation.
  • physical disability preventing safe performance
 

Please note: 

  • 7-8 Participants with mild-moderate COPD/respiratory disease
  • 2-3 Participants with severe COPD

Please advise on referral form if mild, moderate or severe (see below).  

Stage Spirometry  (post-bronchodilator)
  • I – Mild COPD     FEV1 / FVC < 0.7 and FEV1 60% to 80% predicted
  • II – Moderate COPD FEV1 / FVC < 0.7 and FEV1 40% to 59% predicted
  • III – Severe COPD FEV1 / FVC < 0.7 and FEV1 below 40% predicte

Services Offered

Palliative Care After Hours - support, advice and outreach to the residents of Kingaroy and surrounds. 

This service will provide support to patients who require palliative care who wish to remain in their own home. Palliative care nurses will provide support, symptom management, After Hours care planning, outreach to home services and advice in the After Hours period while working closely with the patient's GP to provide support to patients and their carers.

The palliative care service will be provided at no cost to the patient. 

Eligibility Criteria

Provided for individuals who are not able to access any alternative means to receive palliative treatment through their own resources, DVA, MBS or private health insurance.

Hospice Based Palliative Care

Eligibility Criteria

Hospice care is provided to anyone with a life-limiting health condition, who their doctor believes to be in the end-stage of their illness, needing a level of care that cannot be safely provided at home, or if the client does not wish to die at home.

Clients may be referred to the Hospice by themselves, their families and friends, their GP, Community Service Providers and other hospitals. For more information about referring a client please contact our Director of Nursing.

  • Suffer from and be considered to have clinically stable cardiovascular disease (CVD), pre-diabetes (e.g. insulin resistance), type 2 diabetes mellitus (T2DM) and/or metabolic syndrome or a combination of the aforementioned. 
  • Be in the contemplation or action stage for lifestyle modification/behavior change.
  • Be happy to attend the program as part of the group.
  • Be able to be independent within a group setting (i.e. not require consistent 1-on-1 supervision from the clinician).
  • NOT have a diagnosed eating disorder (e.g. anorexia nervosa).

Services Offered

Wound Care – In Hours and After Hours. This service will be provided in the most appropriate setting. Blue are staff will work in conjunction with the patient's GP through a referral process for wound care management. Blue Care nurses will provide wound care treatment as medically required at no cost to the patient. To find out more about this service, click here

Eligibility Criteria

The patient needs to be referred to the wound care program by a medical practitioner due to the acuity and level of complexity of care with the patient requiring additional resources over and above what is usual for community care services. The patient is unable to access any alternative means to receive treatment through their own resources, DVA, MBS or private health insurance.
