Palliative Care After Hours - support, advice and outreach to the residents of Kingaroy and surrounds. This service will provide support to patients who require palliative care who wish to remain in their own home. Palliative care nurses will provide support, symptom management, After Hours care planning, outreach to home services and advice in the After Hours period while working closely with the patient's GP to provide support to patients and their carers. The palliative care service will be provided at no cost to the patient. |
