Driving Quality Improvement in General Practice with PHASES
PHASES is a statewide initiative to prevent heart attacks and strokes through increased screening, recall and treatment. Using clinical tools such as Primary Sense, practice-level quality improvement support and a consumer campaign the goal is to drive earlier intervention, optimise care and reduce the burden of disease on patients and the health system. Join us as we explore how Quality Improvement can help drive this important initiative and prevent cardiovascular disease.
