The 2026 GRADE Guidelines for the Management of Schizophrenia provide updated recommendations for first-line pharmacological treatment. Yet significant gaps between guideline recommendations and contemporary prescribing practice remain, particularly in initial antipsychotic selection and consideration of metabolic and prolactin risk.
