Darling Downs and West Moreton PHN wants to hear about your experiences of healthcare. We regularly TALK ABOUT what is working well and what could be done better, and we also run the Your Experience of Service (YES) survey.

The YES survey helps us to get a better understanding of your experiences of the health services funded by the Darling Downs and West Moreton PHN.

In March and September each year, you may receive an SMS from RHEALTH. This is because you accessed one these services in the prior three months.

The survey is anonymous, and participation is entirely voluntary. Giving feedback (or not) will not affect your ongoing access to services.

Your feedback makes a difference, and we appreciate your time and participation.

The survey will be sent to you via SMS using a unique code.
