The Elder's Health Diary was developed by local Darling Downs Elders with support of Carbal Medical Services and Darling Downs and West Moreton PHN.

The journal supports Elders to keep track of their health information; including contact information, appointments, medications and bush medicine, and blood sugar and pressure record keeping. It also contains helpful information about health services, aged care support and digital health.

Local Aboriginal artist Jenna Bradshaw - a proud Gomeroi and Dhunghutti woman - created the artwork which represents the journey of an Elder moving from place to place within their community.

"My art piece tells a story of our people and how they connect to waterways and country. Water and Country contribute to our healing journey and food source, this will continue with the knowledge passed down from generation to generation. The Healing process plays a big part in our health journey, it’s a feeling like no other to encounter that healing and connection when you are on Country. I feel that this piece was fitting for the Elder Care Journal as our Elder’s are our knowledge keeper’s and our connection to all our cultural heritage. I give thanks to my Elder’s for always passing on knowledge, love and respect."

The journal is available for free to Aboriginal and Torres Strait Islander peoples, Elders, and other community members who would benefit from this helpful resource.
