It’s time to TALK ABOUT death and dying

Talking about death and dying can feel uncomfortable, but it's one of the most important conversations to be had.
Every person, every family, and every community deserves care at the end-of-life care that is compassionate, culturally safe, and shaped by what matters most to them.
Darling Downs and West Moreton PHN CEO, Lucille Chalmers, emphasised the importance of providing a space for meaningful and honest conversations about end-of-life care.
“When we ask people to share their stories, we gain a deeper understanding of what truly supports comfort and dignity at the end of life.
“Listening to community voices is central to our work as we connect and strengthen primary healthcare across the region.”
The PHN is welcoming everyone in the region to join the conversation with our current TALK ABOUT Topic – Death and Dying.
The anonymous survey, which takes 3-5 minutes to complete, is open to anyone who has had a touchpoint with end-of-life healthcare services in the Darling Downs and West Moreton PHN region.
Whether you are a community member, a carer, a health professional, or someone who has walked alongside a loved one, your voice will help inform the PHN’s shared understanding of how everyone can have a dignified experience at the end-of-life.
The survey closes Monday, 4 May 2026.
