Mental Health Clinical Care Coordination (MHCCC) provides support for individuals with severe and persistent mental illness being managed in primary care. MHCCC ensures that people with severe mental illness receive comprehensive, coordinated care to address both their medical and social needs.
The Commonwealth Psychosocial Support Program (CPSP) is designed for individuals with severe mental illness who require short-term assistance to manage daily functioning. It offers non-clinical, community-based support, delivered both one-on-one and in small groups, to help individuals achieve their recovery goals and enhance their overall well-being.
