Mental Health Clinical Care Coordination 

Mental health nurses work with GPs and/or Psychiatrists to provide coordinated clinical care for people with severe and complex mental illness. MHCCC is delivered in line with a GP Mental Health Treatment Plan or equivalent, developed by the GP or Psychiatrist and provides support to review and monitor mental and physical health, monitor medication, liaise with health professionals and link individuals to other needed services. Clients aged 16+. 

Commonwealth Psychosocial Support Program

Non-clinical community-based supports that aim to facilitate recovery for people living with severe, often episodic, mental health challenges. The Program aims to strengthen the capacity of people to live independently and safely in their community, working in partnership with clients alongside their families and carers, as appropriate to achieve recovery goals, through a range of services to help people manage daily activities, rebuild and maintain connections, build social skills and participate in education and employment.
