Across the System: Commissioning, aggregation and a live data clinic
What works for one patient, one practice is only the beginning. This session explores how patient-reported measures can drive improvement across practices, PHNs and the broader health system.
Through our Live Data Clinic, we'll interpret a real, de-identified data set using the questions submitted by participants, demonstrating how patient-reported data can inform commissioning, service redesign and system-wide learning.
Learning outcomes:
- The shift from "What changed for one patient?" to "What can we learn across the system?"
- The Live Data Clinic - bring your questions (not your data) as we interpret a real, de-identified dataset together
- Practice insights into commissioning, data aggregation and building learning health systems that use patient-reported measures to drive improvement
- A conversation with a practice owner or manager and a consumer representative, exploring what it takes to move from collecting data to creating meaningful system change
